L5LuminX
L5LuminX is the form you fill with a client: consent, treatment notes, pictures, and a signature. It lives on the iPhone or iPad in the room. There is no sign-in and no L5 account. L5 does not keep a copy of the form. Appointments, the till, and the client list stay in wellNet.
How it helps the business
- The treatment form stays with the person in the room, on the device they already use.
- One blank form can be shared inside the practice so every phone asks the same questions.
- A finished visit can leave as a passphrase-protected PDF, with pictures on the pages.
- Visit dates can be handed to wellNet as bookings, including mobile and email when the form has them.
- Client records are not uploaded to L5.
- No monthly subscription for the app. A Grok key is optional, and only for scanning a paper vendor card.
- The practice diary, till, and client list live in wellNet. L5LuminX does not replace that book. Export calendar is how a finished form's dates become bookings on the practice phone.

Forms, in flight, and completed
Blank forms sit on the Forms tab. A visit you have not finished stays on In flight, and you can search it. A finished visit moves to Completed. Deleting the app deletes the forms on that device.

Fill the form in the room
Client name, the service today, and the visit date come first. Service chips are shortcuts the salon can edit. Required questions are marked. Sign with a finger or pencil. Take a picture against a text or notes answer. Save halfway and come back later.

A locked PDF for the client
Export PDF asks for a passphrase of at least 20 characters, then shares one PDF. Pictures and the signature are on the pages. There is no separate photo file. Tell the client the passphrase in person, or by message only after the file has gone.

Dates into wellNet
A date on the form can be added, changed, or removed on this device's calendar. Export calendar writes a CSV of those dates, and phone or email when the form has them. On the wellNet practice phone, import that file as bookings. wellNet deletes the file after a successful import.

The same blank form on every phone
Completed visits do not sync between devices. Share form sends the blank template to another L5LuminX in the same practice, without client answers, signatures, or pictures. Use it so the practice asks the same questions. It is not a shop for selling forms.

Make a form
Describe a form in ordinary language and L5LuminX builds the fields on the phone. That does not use an API key. Or build the fields by hand: short text, notes, email, phone, date, choices, yes or no, and signature. A paper vendor card can be photographed, including front and back, when the practice has a prepaid xAI key. Client answers typed later are not sent.

Backup on this device
Backup database writes one file of the forms, visits, pictures, and salon settings, locked with a passphrase. Restore data from file replaces what is on this device. The vendor-card key stays in the Keychain and is not in the file. Apple's iCloud Backup of the whole phone is a separate recovery path. L5LuminX does not use CloudKit.

When a client should leave the device
Delete record removes one completed visit and its pictures. Delete client removes every completed visit, in-flight form, and picture stored under that name. A PDF or calendar file already sent is not brought back. Blank forms stay until you delete the template.
